Since you didn't stressed the setup overhead as you did for simplicity and
speed, I'd like to propose Square

block ciphers around (take a look at
http://www.esat.kuleuven.ac.be/~bosselae/fast.html), and the setup overhead
is very small. It's also exceptionally scalable (probably more than Peter
Gutmann reports for Safer -- smart card code for Square fits half a
kilobyte), and the design is quite simple (if you know what a Galois field
is).
